The last day to purchase will be Tuesday, September 30th.
Prior to purchase, you will be sent the membership consent form via email to review all terms and details in advance of payment.
Once your signed consent form is received, a total of $350 will be charged (your first month’s membership plus the $75 sign-up fee).
A card must be kept on file for automatic recurring monthly payments of $275 for the remaining months.
